L’impact immunologique du cancer avec les thérapeutiques lourdes immuno-suppressives peut être très varié avec comme facteur de risque la leucopénie qui est la cause majeure de l’infection. Les maladies parasitaires chez l’immunocompromis sont causées soit par des organismes habituellement non pathogènes pour le sujet sain ou des organismes éventuellement pathogènes pour le sujet sain mais qui en cas d’immunodépression apparaissent avec un caractère de pathogénicité plus marqué.
Nous présentons un cas d’un enfant neutropénique à la suite d’une chimiothérapie anticancéreuse et qui a présenté un épisode diarrhéique particulièrement sévère, dû à deux parasites intestinaux non opportunistes Entamoeba histolytica et Giardia lamblia. Nous discuterons à travers ce cas la fréquence et la sévérité de l’infestation du patient immuno-compromis par ces deux parasites et l’intérêt probable du traitement systématique de ces patients par un médicament antiparasitaire actif avant de les mettre sous chimiothérapie immunosuppressive.
Immunological impact of cancer with immunosuppressive drugs can be variable with leucopenia as a major factor of risk. Parasitic diseases in immunocompromised are caused either by organisms usually non pathogenic for the healthy subject or of possibly pathogenic organisms for the healthy subject but which in the event of immunodepression appear with a more marked character of pathogenicity.
We present a case of a neutropenic child who followed an anti-cancer chemotherapy and who presented a particularly severe diarrheal episode, attached to two non-opportunist intestinal parasites: Entamoeba histolytica and Giardia lamblia. We will discuss through this case the frequency and the severity of the infestation of the immunocompromised patient by these two parasites and the probable interest of systematic treatment of these patients by a anti parasitic drug before putting them under immuno suppressive chemotherapy.
