Le syndrome de Fahr est une entité anatomo-clinique rare, caractérisée par des calcifications bilatérales et symétriques situées au niveau des noyaux gris centraux. Le syndrome de Fahr est souvent associé à une dysparathyroïdie (principalement une hypoparathyroïdie). Il se traduit par un grand polymorphisme clinique avec prédominance des manifestations neuropsychiatriques. Les mécanismes physiopathologiques qui expliquent la survenue de telles calcifications intracérébrales restent encore mal élucidés.
Nous rapportons le cas d’une femme (âgée de 37 ans), consultant pour crises épileptiques évoluant depuis 4 ans et résistantes aux traitements antiépileptiques. La tomodensitométrie cérébrale montrait des calcifications des noyaux gris centraux et des noyaux dentelés. Le bilan biologique était en faveur d’une hypoparathyroïdie. Le diagnostic du syndrome de Fahr est retenu et un traitement par calcium et vitamine D est instauré. L’évolution est favorable.
Notre observation souligne l’intérêt de rechercher systématiquement une anomalie du métabolisme phosphocalcique, devant des calcifications intracérébrales, car l’évolution sans traitement, peut engager le pronostic vital du malade.
Fahr’s syndrome is defined by the presence of striopallidal notched bilateral and symetric calcifications at the base of the skull. It preferentially occurs in patients with dysparathyroidia (especially to hypoparathyroidism). This rare disorder is characterized by its clinical polymorphism but includes in the first line neuropsychiatric manifestations, the pathophysiological mechanisms that explain the occurrence of such intracerebral calcification remain poorly understood.
We report the case of a woman (aged 37), who has been monitoring for partial seizures lasting for four years and which are resistant to antiepileptic drugs. Brain computed tomodensitometry identified intracranial calcifications. The tests demonstrated hypoparathyroïdism. The diagnosis of Fahr’s syndrome has been accepted and a therapy with calci-vitamin therapy was instituted. The evolution was favorable.
Our observation stresses the importance of systematically seeking and abnormal mineral metabolism in front of intracerebral cacification, because the evolution without treatment can be life threatening for the patient.
