Introduction : Il existe une association forte entre cancer et maladie veineuse thromboembolique (MVTE). Le cancer bronchique est l’une des néoplasies les plus pourvoyeuses de MVTE à laquelle le pneumologue est fréquemment confronté à la fois en termes de diagnostic, de prévention et de traitement.
Objectifs de l’étude : Etudier les caractéristiques épidémiologiques, les présentations cliniques ainsi que les particularités thérapeutiques des cancers broncho-pulmonaires associés à une MVTE et identifier parmi les patients cancéreux, les patients à haut risque de développer une thrombose.
Patients et méthodes : Etude rétrospective, portant sur les patients suivis pour un cancer broncho-pulmonaire confirmé et qui ont présenté une MVTE au moment du diagnostic du cancer ou au cours de son évolution.
Résultats : Nous avons recensé 34 patients avec un sex-ratio à 7.5 et un âge moyen de 56 ans. La plupart des patients (82%) a présenté une MVTE au cours des 6 premiers mois du diagnostic du cancer. La répartition des accidents thromboemboliques était une embolie pulmonaire dans 44% des cas et une thrombose veineuse profonde dans 56% des cas. L’adénocarcinome était le type histologique prédominant (60%). Au moment de la découverte de la thrombose, 83% des patients étaient au stade métastatique. Le traitement initial de l’épisode thromboembolique a consisté en une héparine non fractionnée chez 3 patients et une héparine de bas poids moléculaire chez 30 patients. Le traitement anticoagulant au long cours reposait sur une héparine de bas poids moléculaire pour 13 patients et sur les anti-vitamines K pour les autres. La survenue de l’hémoptysie chez les patients sous anti-vitamine K était 2 fois supérieure que ceux qui recevaient une héparine de bas poids moléculaire. Quatre patients avaient une récidive thrombo-embolique. La médiane de survie était de 51 jours par rapport au diagnostic de la MVTE.
Conclusion : A côté des facteurs de risque classiques de la thrombose veineuse, de nombreux facteurs de risque spécifiques influencent sa survenue chez les patients cancéreux : localisation, histologie, stade, traitements et ancienneté de la maladie néoplasique.
Introduction: There is a strong association between cancer and Venous Thrombo Embolism (VTE). Lung cancer is one of the most neoplasms providers of VTE to which physicians are frequently confronted both in terms of diagnosis, prevention and treatment.
Aims of the study: To study the epidemiology, clinical presentations and therapeutic particularities of broncho pulmonary cancers associated with VTE and identify among cancer patients, those at high risk of developing thrombosis.
Patients and methods: Retrospective study on patients followed for a broncho-pulmonary cancer and who developed a VTE at diagnosis of cancer or during its evolution.
Results: We identified 34 patients with a sex-ratio at 7.5 and an average age of 56 years. Most patients (82%) developed VTE during the first 6 months of cancer diagnosis. The distribution of thromboembolic events was a pulmonary embolism in 44% and deep venous thrombosis in 56% of cases. Adenocarcinoma was the predominant histological type (60%). Upon discovery of thrombosis, 83% of patients were metastatic. The initial treatment of thromboembolic episode consisted of unfractionated heparin in 3 patients and a low molecular weight heparin in 30 patients. The long term anticoagulant treatment was based on a low molecular weight heparin for 13 patients and anti-vitamin K for the others. The occurrence of hemoptysis in patients with anti-vitamin K was 2 times higher than that receiving low molecular weight heparin. Four patients had recurrent thromboembolism. Median survival was 51 days from the diagnosis of VTE.
Conclusion: Besides the traditional risk factors of venous thrombosis, many specific risk factors influence its occurrence in cancer patients : location, histology, stage, treatment and age of the neoplasic disease.
