Introduction : La rhinosinusite chronique invasive aspergillaire d’allure pseudo-tumorale est une infection rare, de diagnostic difficile et survenant habituellement chez les sujets immunodéprimés.
Observation : Patiente de 54 ans, avec antécédents de néoplasie du sein droit traité il y a 5 ans. Elle consultait pour une obstruction nasale gauche. La rhinoscopie objectivait une masse polypoïde de la fosse nasale gauche. Le Blondeau scanner montrait un comblement tissulaire du sinus maxillaire gauche avec fines calcifications associé à envahissement de la fosse nasale homolatérale. L’exploration chirurgicale par méatotomie montrait une tumeur fragilisant les parois sinusales retirée en totalité. L’examen anatomopathologique avait permis le diagnostic de rhinosinusite chronique invasive aspergillaire. La malade était mise sous une antibioprophylaxie, traitement anti-inflammatoire et traitement antifongique avec surveillance clinique et biologique. Les suites opératoires étaient simples.
Discussion : Le caractère varié et non spécifique des signes cliniques et le caractère invasif pseudo-tumoral orientant plutôt vers un processus malin sont à l’origine de retard et difficultés diagnostiques. La prise en charge thérapeutique doit être rapide afin d’éviter des complications de pronostic redoutable.
Conclusion : Infection rare dont le diagnostic repose sur l’examen anatomopathologique et mycologique. Le pronostic particulièrement grave de cette forme justifie une prise en charge thérapeutique rapide.
Introduction: Chronic invasive rhinosinusitis aspergillosis with pseudo-tumoral presentation is a rare infection of difficult diagnosis, usually occurring on a defective predisposed ground.
Observation: Fifty four year-old patient, with a history of a 5 years ago treated right mammary cancer. She consulted for a left nasal obstruction. The rhinoscopy objectified a polypoid mass occupy-ing the higher half of the left nasal fossae. The Blondeau scanner showed a tissular process of the left maxillary sinus with fine calcifications associated with the homolateral nasal fossae invasion. Surgical explo-ration by meatotomy highlighted a tumour associated with an embrittlement of the sinusal walls. This tumour was friable, partially necrosed and withdrawn in totality. The anatomo pathologic examination had allowed the diagnosis of chronic invasive pseudotumoral sinusitis aspergillosis. The patient was treated by antibioprophylaxy, anti-inflammatory drugs treatment and antifungal treatment with clinical and biological monitoring. After a good postoperative recovery, a monthly follow-up with radio-logical control was founded with a 3 months passing.
Discussion: The varied and non-specific character of the clinical signs are the cause of delay and diagnostic difficulties. Therapeutic support must be quick in order to avoid complications of daunting prognosis.
Conclusion: Rare infection which diagnosis is based on the mycological and pathological examination. The particularly serious prognosis for this form warrants a rapid therapeutic support.
