L’hypertrophie ventriculaire gauche (HVG) est un facteur majeur de morbidité et de mortalité cardiovasculaire chez l’hémodialysé chronique.
Dans une étude transversale réalisée en juin 2012 au sein de service de néphrologie, hémodialyse du 1er Centre médico-chirugical (1er CMC) d’Agadir, Maroc, nous avons étudié les facteurs statistiquement corrélés à l’HVG. Vingt-trois hémodialysés ont été inclus dans l’étude (14 femmes et 9 hommes). Les patients étaient dialysés au moins 12 heures par semaine. L’âge moyen était de 45,6 ans [22-73 ans]. L’index de masse corporelle moyen (BMI) est de 25,5 kg/m2. La moyenne d’ancienneté en hémodialyse était de 54,6 mois [8-144 mois]. La néphropathie d’origine indéterminée est la plus fréquente 30,4% suivie par l’étiologie diabétique 17,4%. La prise de poids interdialytique moyenne est de 2,10 ± 1,20 kg (1,10 -5 kg) et 30,7% des patients étaient hypertendus. La valeur moyenne du Kt/V est de 1,15 ± 0,13 (0,90-1,30).
Sur le plan cardiologique, 9 patients (39,1%) avaient une hypertrophie ventriculaire gauche à l’échographie transthoracique. Les patients présentant une hypertrophie ventriculaire gauche étaient plus hypertendus et présentaient une moyenne de prise de poids interdialytique plus importante et statistiquement significative que les patients n’ayant pas d’HVG (P=0,02). Par contre il n’y a pas d’association significative entre l’HVG et l’anémie. Ceci souligne l’intérêt d’optimisation de la dialyse par l’évaluation régulière de l’ultrafiltration permettant le maintien d’un poids sec optimal.
Left ventricular hypertrophy (LVH) is a major factor of morbidity and cardiovascular mortality in chronic hemodialysis.
In a cross-sectional study in June 2012 in the Division of Nephrology, Hemodialysis first medical-surgical center (1st MSC) Agadir, Morocco, we studied the factors statistically correlated with LVH. Twenty three hemodialysis patients were included in the study (14 women and 9 men). Patients were dialyzed at least 12 hours per week. The average age was 45.6 years [22-73 years]. The mean body mass index (BMI) was 25.5 kg/m2. The average length of hemodialysis was 54.6 months [8-144 months].
Nephropathy of unknown origin is the most common (30.4%) followed by diabetic nephropathy (17,4%). Interdialytic weight gain average was 2.10 ± 1.20 kg (1.10 -5 kg) and 30.7% of patients were hypertensive. The mean Kt/V was 1.15 ± 0.13 (0.90 to 1.30).
Nine patients (39.1%) had left ventricular hypertrophy on transthoracic echocardiography. Patients with left ventricular hypertrophy were more hypertensive and had a largest mean interdialytic weight gain nd statistically significant than patients with no LVH (P = 0.02). However there was no significant association between LVH and anaemia. This underlines the importance of optimization of dialysis by regular evaluation of ultra filtration for the maintenance of an optimal dry weight.
