Introduction : La polyglobulie vraie est un syndrome myélo-prolifératif caractérisé par un risque accru de saignement et d'événements thrombotiques. L'infarctus du myocarde reste la première cause de mortalité chez ces patients. Certaines complications thrombo-emboliques et/ou hémorragiques liées à la technique de revascularisation coronaire ont été signalées dans la littérature.
Observation : Nous rapportons un cas d'un patient de 58 ans qui présente un syndrome coronarien avec sus-décalage de ST en circonférentiel revascularisé par streptokinase avec succès chez qui une polyglobulie vraie a été retrouvée retenue devant le taux d'hémoglobine à 21 g/dl et la mutation du gène V617F JAK2 ainsi qu'un taux d'érythropoïétine bas. La coronarographie a montré une lésion significative de l'IVA. Vu le terrain d'hypercoagulabilité la décision de ne pas stenter la lésion coupable a été prise après une discussion pluridisciplinaire (cardiologue, hématologue, cardiologue interventionnel et chirurgien cardiovasculaire) et une épreuve d'effort faite à J5 post-infarctus se révélant négative. Le patient a reçu une thérapeutique cytoréductrice à base de saignés et d'hydroxyurée. Le patient était déclaré sortant au 10ème jour de son admission et mis sous hydroxyurée 1500mg/j, aspirine 100mg/j, Clopidogrel 75mg/j. Après deux ans de suivi, on ne note aucune récurrence d'événement thrombo-embolique.
Conclusion : Nous soulignons, à travers ce cas, la complexité de la gestion des syndromes coronariens avec sus-décalage du ST secondaires à une polyglobulie vraie d'où l'intérêt d'une bonne évaluation de la balance "risque embolique-risque hémorragique" pour chaque patient et insister sur la thérapeutique cytoréductrice et une anti-agrégation plaquettaire adéquate ainsi qu'un contrôle optimal du taux d'hématocrite comme stratégie optimale de prévention secondaire.
Introduction:Polycythemia vera is a myeloproliferative disorder that is characterized by an increase in phenotypically normal RBCs, which leads to hyperviscosity and an increased risk of bleeding and thrombotic events. Myocardial infarction, remain the primary cause of mortality. Some complications (thrombo-embolic and/or hemorragic) related to the coronary reperfusion were reported in the literature.
Case presentation: We report a case of a 58-years-old patient that presenting a ST elevation myocardial infarction in circumferential territory thrombolised by streptokinase with success. The investigations revealed a Polycythemia vera, the coronary angiography showed a significant lesion on the LAD, the decision to do not stent the LAD lesion was made after a multidisciplinary discussion (cardiologist, hematologist, interventional cardiologist) and a negative stress test, the patient was discharged at day 10th of admission under hydroxyurea 1500 mg/d, aspirin 100 mg/d, clopidogrel 75 mg/d. After two years of follow-up, the patient was still free of thromboembolic events.
Conclusion: We highlight, through this case, the complexity of the management of ST elevation myocardial infarction due to Polycythemia vera, where the coagulation balance must be taken into account. The cytoreductive therapy and anti-aggregation therapy may be the optimal secondary prevention option for such patients.
