Résumé

Ce rapport de cas décrit le traitement orthodontique facilité par corticotomie alvéolaire chez une patiente adulte présentant une béance antérieure, une interposition linguale et une constriction transversale du maxillaire, avec présence des corridors buccaux. Le protocole final de traitement a consisté en l’utilisation d’appareillage multi- attaches associé à un disjoncteur et combiné à une corticotomie sélective maxillaire et mandibulaire par ostéo-perforations au moyen d’un appareil piézo-électrique. Les résultats ont été obtenus après 6 mois de traitement actif, se traduisant par une amélioration nette du sourire avec réduction des corridors buccaux et fermeture de la béance avec un recouvrement et une intercuspidation adéquate. Ces corrections sont restées stables à 3 ans en post-contention. Le recours à cette approche combinant traitement orthodontique et corticotomie sélective a permis une diminution significative de la durée de traitement et une stabilité de la correction de la béance antérieure. Ainsi, cette approche de traitement peut constituer une option à intégrer dans le panel des stratégies thérapeutiques de gestion de la béance antérieure, surtout dans le contexte spécifique des cas limites, de patients adultes réfractaires à la chirurgie et désireux d’un traitement de courte durée.

Abstract
Orthodontic treatment of an anterior open bite facilitated by piezo-osteo-perforations

This case report documents the orthodontic treatment of an adult patient with anterior open bite, tongue interposition and transverse constriction of the maxilla, associated with the presence of buccal corridors, performed based on alveolar corticotomy. The treatment protocol involved the use of a multi-bracket appliance combined with a disjunctor and selective maxillary and mandibular corticotomy through osteo-perforations using a piezoelectric device. Results were obtained after 6 months of active treatment, resulting in a significant improvement in the patient’s quality of life. These corrections remained stable three years after retention. The use of this approach, combining orthodontic treatment and selective corticotomy, resulted in a considerable reduction in treatment time and stability in anterior bite correction. As such, this therapeutic approach may be regarded as a viable option in the repertoire of therapeutic strategies for the management of anterior bite, particularly in borderline cases, in adult patients resistant to surgery and in those who desire a shorter treatment duration.
