Résumé

Introduction : Les limitations d’ouverture buccale peuvent représenter une contre-indication de chirurgie implantaire en zone postérieure. Cependant notre cas clinique avait un large édentement et une perte de la fonction masticatoire et pour le réhabiliter et l’aider à retrouver la mastication avec efficacité, nous n’avions pas le choix que de tenter la pose implantaire.
Cas clinique : Nous avions reçu un patient de 35 ans victime des terroristes de Boko Haram. Il avait un traumatisme balistique au maxillaire et à la mandibule. Il présentait une perte importante de tissus mous et de tissus durs au maxillaire et à la mandibule. Il avait une ouverture buccale limitée à 26 millimètres. Nous avions utilisé des techniques chirurgicales adaptées à nos moyens pour une prise en charge implantaire, notamment l’utilisation adaptée d’écarteurs buccaux pour avoir un accès buccal ; puis une technique de régénération osseuse guidée pour augmenter la quantité osseuse ; des poses implantaires sans guide chirurgical ; une vestibuloplastie pour augmenter de la gencive kératinisée ; une technique de lambeau positionné coronairement associé à une greffe de tissu conjonctif pour un recouvrement radiculaire. Nous avions pu au bout de 16 mois de traitement avoir 8 implants ostéo-intégrés et des conditions gingivales correctes pour entamer la réhabilitation implanto-prothétique.
Conclusion : Les limitations d’ouverture buccale notamment en cas de microstomie sévère rendent difficiles les chirurgies implantaires surtout en zone postérieure, mais pas impossibles. Pour réussir, Il faut s’adapter aux difficultés en utilisant des techniques chirurgicales judicieusement choisies.

Abstract
Surgical-implant-prosthetic rehabilitation of ballistic trauma with limitation of mouth opening: A case report

Introduction: Limited mouth opening can represent an absolute contraindication for implant surgery in the posterior region. Our clinical case involved a significant edentulous area with loss of masticatory function, and to help the patient regain masticatory function with improved proprioception, we had no choice but to attempt the placement of implants despite the limitation in mouth opening.
Clinical case: We received at the Niamey military dental service a 35-year-old patient who was a victim of Boko Haram terrorists. He had ballistic trauma to the maxilla and mandible. He presented a significant loss of musculofacial tissue, maxillary bone tissue, keratinized gum, and several teeth in the maxilla and mandible. He had a mouth opening limited to 26 millimeters. We used surgical techniques adapted to our means for implant treatment, in particular the adapted use of oral retractors to have oral access; then a guided bone regeneration technique to increase bone quantity; implant placements without a surgical guide; a vestibuloplasty to increase keratinized gum; a coronally flap placement technique associated with a connective tissue graft for root coverage. After 16 months of treatment, we were able to have 8 osseointegrated implants and correct gingival conditions to begin implant-prosthetic rehabilitation.
Conclusion: Limitations in mouth opening, particularly in cases of severe microstomia, makes implant surgeries difficult, especially in the posterior area, but not impossible. To succeed, we must adapt to difficulties by using judiciously chosen surgical techniques.
