Résumé

Cinq ans après une extraction dentaire, malgré la prescription d'antibiotiques, il se produit une ostéite nécrosante de l'angle mandibulaire, avec importante asymétrie faciale. C'est une complication rare pour une extraction. Le contexte socio-culturel et économique aidant, la patiente a longuement fréquenté les tradipraticiens, sans résultats. Le traitement a consisté en une incision extra-orale, une ablation de séquestres et drainage de la poche purulente puis, blocage inter-maxillaire dynamique par des attèles en résine. En deux mois l'asymétrie faciale a été jugulée. Du point de vue diagnostic étiologique, seul l'interrogatoire permet de dire que l'extraction était traumatique, posant ainsi le problème de la responsabilité chirurgicale et du suivi post-opératoire, liés aux conditions cliniques dans lesquels les soins sont administrés.

Abstract
Mandibular angle necrosis with facial asymmetry as complication tooth extraction : report of a case

Five years after a tooth extraction, a bone necrosis of the mandibular angle happened, without consideration to the antibiotics. Such complication is rare. Living in poor socio-cultural and economic conditions, the patient tried to find help near tradipractionners, prayers groups, without any result. The treatment consisted to an extra oral incision, helping to eliminate bone sequestra, and the liquid around after a dynamic bimaxillary linkage by resin links was settled. About aetiology, only the questioning allowed to say that extraction was traumatic, putting in front of the surgical responsability and post surgical survey.
