Portant sur 309 patients (171 femmes et 138 hommes) consultés par 15 odonto-stomatologistes de Yaoundé pendant un an, cette étude avait pour objectif l’évaluation de la mise en œuvre d’une antibiothérapie en pratique quotidienne. Nous avons enregistré 10 affections bucco-dentaires les plus fréquentes, ayant nécessité une antibiothérapie curative, représentant 87 % des prescriptions. Une antibiothérapie prophylactique a été prescrite dans trois pour cent des cas, chez des patients porteurs de pathologies générales (cardiopathies, diabète…) ou de fractures mandibulaires. Dans 10 % des cas, une antibiothérapie de couverture a été instituée lors des extractions simples de dents non infectées chez des sujets sains. La durée de l’antibiothérapie était courte dans 48 % des cas (moins de huit jours) et dans 32 % des cas les doses étaient infrathérapeutiques. Nous avons relevé trois pour cent de prescriptions d’antibiotiques (sulfamides, phénicolés…) inadaptés aux germes habituels de la cavité buccale. Des génériques ont été prescrits dans 12 % des cas. Les antibiotiques les plus prescrits étaient les amoxicillines, les macrolides et les céphalosporines. Seule une bonne connaissance des familles d’antibiotiques avec leurs indications, sont de nature à éviter certaines maladresses. La prescription des médicaments génériques dans notre environnement économique si difficile devrait être plus courante.

Abstract

Based on 309 patients (171 women and 138 men), consulted by 15 dental surgeons of Yaounde during one year, the aim of this study was the evaluation of bringing into use an antibiotherapy in daily practice. A questionnaire on the subject was distributed to practitioners. We registered 10 most frequent mouth dental diseases that needed curative antibiotherapy, being 87 % of prescriptions. A prophylactic antibiotherapy has been prescripted in three per cent of cases to patients carriers of general pathologies (cardiopathies, diabetes…) or mandibular fractures. In 10 % of cases, a covering antibiotherapy has been established during simple extractions of teeth without infections, from healthy patients. In 48 % of cases the duration of antibiotherapy was short (less than eight days) and in 32 % of cases, the quantities were infratherapeutic. There were three per cent of prescriptions concerning antibiotics (sulfonamides, phenicoles…) unsuited to usual germs of mouth cavity. Generics drugs have been prescribed in 12 % of cases. The more prescribed antibiotics were amoxicillins, macrolides and cephalosporines. Only a best knowledge of antibiotics’ families and their indications are able to avoid clumsiness. At last generic drugs prescription in our difficult economical environment should be common.
