Résumé

Une enquête épidémiologique menée à Abidjan dans trois écoles publiques (primaire, collège et lycée) a permis de déterminer la prévalence des affections rhinopharyngées en présence d’anomalies orthodontiques. C’est une étude prospective, transversale menée conjointement par deux équipes de spécialistes en ORL et en ODF sur un échantillon d’enfants africains mélanodermes scolarisés âgés de 5 à 21 ans. Les résultats ont montré la présence de malocclusions dans 73,30 % des cas (n= 220). Les anomalies rhinopharyngées représentent 38,3 % (n = 115). Chez les sujets porteurs d’une dysmorphose alvéolo-dentaire les anomalies rhino-pharyngées sont de 48,30 % et elles sont dominées par la rhinite chronique allergique (40 %) suivie des amygdalites hypertrophiques obstructives (16,5 %). Cependant, ces états qui occasionnent une obstruction nasale entraînant certes une respiration buccale, ne sont pas forcément source de perturbation alvéolo-dentaire. Nous ne pouvons donc pas affirmer sans équivoque un lien entre les anomalies orthodontiques et les affections rhinopharyngées (p > 0,05). Elles sont néanmoins autant d’indications pour interroger le patient sur d’autres symptômes et référer à un spécialiste ORL. Notre enquête étant limitée à l’étude occlusale, il nous semble indispensable de poursuivre ces analyses pour dépister la prédisposition éventuelle de certains patients à développer des dysmorphoses dento-squelettiques en présence d’affections rhinopharyngées.

Abstract

An epidemiological survey of randomly selected school children led to Abidjan in three public schools made it possible to determine the prevalence of the rhinopharyngeal diseases in presence of malocclusions. It is an exploratory study jointly undertaken by two teams of specialists in ORL and Orthodontics within a sample of African schoolchildren old of 5 to 21 years. The results showed the presence of malocclusions in 73,30% of the cases (N = 220). The rhinopharyngeal diseases account for 38,3% (N = 115). At the subjects carrying a malocclusion the rhinopharyngeal diseases are 48,30% and they are dominated by the allergic chronic rhinitis (40%) followed obstructive hypertrophic tonsillitis (16,5%). However, these states which cause certainly a nasal obstruction involving an oral breathing do not cause inevitably malocclusion. We cannot thus affirm unambiguous a bond between malocclusions and rhinopharyngeal diseases (p > 0,05). They are nevertheless as many indications to question the patient on other symptoms and to refer to an ORL specialist. Our investigation being limited to the occlusal study, it seems essential to us to continue these analyses to detect the possible predisposition of certain patients to develop dento-skeletal anomalies in the presence of rhinopharyngic diseases.
