Résumé

La réalisation d'une prothèse fixée entraîne une action directe sur le rebord marginal de la gencive. Elle est soumise à des impératifs d'exécution dont l'un des objectifs est de respecter le parodonte avec une grande rigueur. Dans le cas contraire, elle peut devenir un agent d'agression majeur, source d'échecs prothétiques.
Nous avons mené ce travail pour évaluer dans le cadre d'un programme pédagogique opérationnel de prothèse conjointe, les risques parodontaux superficiels liés aux préparations des dents-piliers et à la chaîne prothétique. Sur un échantillon de 50 patients pris au hasard dans la consultation de prothèse de l'Institut d'Odontologie et de Stomatologie (I.O.S.) de Dakar, au parodonte initialement sain, nous avons examiné des lésions inflammatoires de parodonte superficiel du début à la fin du traitement, en nous aidant de l'indice parodontal de Russel. L'ensemble des scores individuels, récapitulés sur une fiche collective ont été soumis à l'analyse statistique du test d'indépendance du khi2. La population consultant en prothèse fixée à l'Institut d'Odontologie et de Stomatologie de Dakar est relativement jeune : 33 ans avec un sex-ratio de 1,17 et présente une atteinte parodontale modérée (60 % des cas) ou sévère (40 % des cas) liée de manière significative au type de prothèse et à la morphologie axiale et occlusale des pièces prothétiques.
Cette étude montre que la réhabilitation prothétique dans le cadre d'un enseignement clinique à l'I.O.S. de Dakar, comporte des risques d'agression du parodonte relativement élevé qu'il faudrait minimiser en posant mieux les indications de Richmond et en apportant plus de rigueur dans l'enregistrement et le transfert de l'occlusion de patient et dans la conception au laboratoire des pièces prothétiques.

Abstract

The realisation of a bridgework involves a direct action on the gingival marginal edge. It is submitted to peremptoriness of execution that one of the objectives is to respect the periodontium with a great rigour. In the opposite case it can become a greater aggression agent, source of prosthetic failures we carried out this report in order to evaluate in the framework of a pedagogic operational programme of crown and bridge prosthesis, the superficial periodontal risks linked to the preparation of the abutment teeth and to the prosthetic chain.
We examined a randomised sample of 50 patients chosen from the Dakar LOS prosthetic consultation. On the periodontium initially sound we noted inflammatory lesions of the superficial periodontium from the beginning to the end of the treatment we used the Russel periodontal index. The whole individual scores summarised on a collective chart have been submitted to the statistical analysis of the independence by using the chi-square test. The population consulting in bridge work at the Dakar LOS is relatively young, 33 years in a average with a sex ratio of 1.17 and it shows a moderate (60 % of the cases) and severe periodontal seizure (40 % of the cases) dealing significantly with the type of prosthesis and the axial and occlusal morphology of the prosthesis pieces.
This study shows that prosthesis rehabilitation in the frame work of a clinical training at the Dakar I.O.S includes relatively high risks of periodontium aggression that we would minimise by setting more correctly the indications of the bridge work and by bringing more rigour to the record and the postpone of the patients occlusion in the laboratory conception of the prosthesis pieces.
