Résumé

L’objectif de cette étude est d’évaluer un nouveau test pour le diagnostic de la candidose buccale (test fungi-dent color®) dans une population de personnes âgées institutionnalisées. Les levures font partie de la flore humaine normale et vont exploiter les situations de faiblesse de l’organisme hôte. Les études épidémiologiques des mycoses buccales ont conclu que l’espèce prédominante était Candida albicans.
Matériel et méthode : Cette étude concerne 73 patients hospitalisés à court ou long terme dans le service de gériatrie du CHU de Montpellier. Après un examen bucco-dentaire, la présence de Candida est recherchée : la méthode diagnostique par le test fungi-dent color® est comparée d’une part au test classique de culture en laboratoire (méthode de référence) et d’autre part au diagnostic clinique par simple examen visuel.
Résultats : La sensibilité du test fungi-dent color® est de 64,6% (intervalle de confiance : 53,6% - 75,6%) et sa spécificité de 96,0% (IC : 92,0% - 100%), alors que l’examen visuel ne révèle que 22.5% des candidoses présentes. L’aire sous la courbe ROC est de 0,81. De plus, le pH moyen est significativement plus élevé lorsque le diagnostic de candidose est établi. (6,15 vs 5,78, p = 0,007).
Conclusion : L’analyse statistique montre que le test fungi-dent color® peut apporter une aide significative dans le diagnostic des candidoses buccales, avec des performances en termes de fiabilité et de rapidité bien supérieures à celles habituellement obtenues à la suite d’un simple examen clinique de routine.

Abstract
Evaluation of a new test for candidiasis diagnosis in elderly people

Objective: The purpose of this study was to evaluate a new test for candidiasis diagnosis (fungi-dent color® test) in an institutionalised elderly population.
Background: Yeasts normally exist in the human body flora and exploit a situation of weakness in the host. Epidemiologic analyses of yeast infection in the oral cavity showed that the most frequently involved species were Candida albicans.
Materials and methods: This study included 73 persons in the short-term and long-term geriatric department of the Montpellier hospital. Oral examinations were performed and the presence of Candida was evaluated: the fungi-dent® Color diagnostic method was compared both to the classical culture analysis method, considered as the gold standard, and to the visual clinical diagnostic.
Results: The fungi test yielded a sensitivity value of 64.6% (CI: 53.6% - 75.6%) and a specificity of 96.0% (CI: 92.0% - 100%), whereas the diagnostic method through clinical examination disclosed only 22.5% candidiasis. The area under the ROC curve was equal to 0.81. The mean pH value was significantly higher when candidiasis was present (6.15 vs 5.78, p = 0.007).
Conclusion: Statistical analyses showed that the Fungi-dent-color® test was eligible to be validated for community use, with better characteristics than a routine clinical diagnosis and with a reasonably rapid and reliable diagnostic outcome.
