Résumé

Contexte : L’évaluation de la santé bucco-dentaire a évolué avec le nouveau concept de la maladie carieuse. Il est nécessaire de connaitre les concordances entre les anciennes et les nouvelles méthodes d’enregistrement des lésions carieuses.
Objectif : Identifier les codes d’ICDAS II à employer pour enregistrer la composante C de l’indice CAO, définit par l’OMS en 1997.
Méthode : Une revue de la littérature des articles publiés entre janvier 2002 et janvier 2012 a été réalisée en utilisant “ICDAS” comme mot-clé pour la recherche électronique. Seules les études épidémiologiques qui ont employé ICDASII comme critère d’évaluation, calculé les indices CAO et ont donné les codes d’ICDAS II utilisés pour le calculer, ont été incluses.
Résultats : Quatorze études ont répondu aux critères d’inclusion. Les CAO calculés étaient le C3-6AO (10 études), C4-6AO (4 études) ou C5-6AO (3 études). La composante C faisait référence aux lésions carieuses cavitaires (7 études) ou aux lésions dentinaires (7 études), et il n’y avait aucun consensus sur les codes d’ICDAS II employés pour les définir. Seules les lésions ICDAS II 5-6 étaient unanimement considérées comme carieuses. En revanche, l’utilisation des codes ICDAS II 3-4 ne faisait pas l’unanimité. La seule étude épidémiologique qui avait comparé les deux méthodes a montré que C3-6 correspondait à la composante C de l’indice CAO défini par l’OMS.
Conclusion : Il y avait des désaccords sur les codes d’ICDAS II à employer pour le calcul du CAO. Ainsi pour comparer les valeurs de CAO de différentes études, il est nécessaire de se référer au seuil de diagnostic carieux utilisé, qui doit être le même.

Abstract
DMF from WHO Basic Methods to ICDAS II Advanced Methods : a systematic review of literature

Background: Evaluation of caries status has changed with emergence of modified ways of managing the condition. There is a need to assess the relationship between the old and new methods of registering caries.
Objective: To identify the ICDAS II codes to be used to record the D-component of the DMF index as defined in the WHO Basic Methods, 1997 publication.
Method: A review of literature published between January 2002 and January 2012 was undertaken using “ICDAS” as keyword in an electronic search. Only epidemiological studies that used ICDAS II as an evaluation criterion calculated the DMF indices and gave the ICDAS II codes for the diagnosis of caries lesions, were included.
Results: Fourteen studies met the inclusion criteria. The DMF designations that corresponded with the WHO definition were D3-6MF (10 studies), D4-6MF (4 studies) or D5-6MF (3 studies). The D-component referred to cavitated carious lesions (7 studies) or dentine caries (7 studies), but there was no consensus on the ICDAS II codes that are used to define them. Only the ICDAS II codes 5 and 6 had unanimous support; they were always counted as “Caries”, but there was less certainty for codes 3 and 4. The only study on fields that compared both methods showed D3-6 to be the always associated with the D-component of the DMF index as defined in the WHO Basic Methods.
Conclusion: There was disagreement of the ICDAS II codes to be used for the DMF calculation; and when there was a need to compare DMF values between studies, the diagnosis threshold should be verified to be the same.
