Résumé

Introduction : L'objectif de cette étude a été d'évaluer les connaissances, attitudes et pratiques (CAP) des dentistes marocains sur l'allergie au Nickel (Ni) au cours du traitement orthodontique.
Population et méthodes : L'ensemble des dentistes de deux villes marocaines a été contacté (n = 553) et un questionnaire a été distribué à ceux qui ont attesté d'une activité orthodontique partielle ou exclusive (n = 202). Il leur a été demandé de spécifier le type de réactions allergiques observées et les alliages utilisés. Les CAP concernant l'étiologie, la prévention et la conduite à tenir devant les patients allergiques ont également été étudiés.
Résultats : Vingt-deux pour cent des dentistes ont observé au moins une réaction d'allergie au Ni avec une prédominance de l'incrimination des appareils orthodontiques fixes. Parmi les différentes localisations rapportées, 18% ont concerné la zone muqueuse et la peau du visage proche de la zone de contact et moins de 5% se situaient à des localisations à distance. Seuls 34% des praticiens connaissaient l'étiologie de l'allergie au Ni. 94% des praticiens ont avancé le recours au changement du biomatériau et seulement 2% ont suggéré la réalisation de tests épi cutanés devant la suspicion d'allergie au Ni.
Conclusion : Les alliages orthodontiques renfermant du Ni continuent d'être largement utilisés, même en cas de suspicion de terrain allergique. Dans ces cas, il est important de minimiser les facteurs de corrosion et de privilégier les dispositifs nickel free.

Abstract
Evaluation of the knowledge attitude and practices of orthodontists about allergy to Nickel during orthodontic treatment

Introduction: The aim of this study was to evaluate knowledge, attitudes and practices (KAP) of Moroccan dentists about allergy to Nickel (Ni) during orthodontic treatment.
Population and methods: Dentists of two Moroccan cities (n = 553) were contacted and a questionnaire was distributed to those who have certified of partial or exclusive practice of orthodontics (N = 202). They were asked to specify the adverse reactions observed and the alloys frequently used. KAP about etiology, prevention and therapeutic management of allergic patients were also addressed.
Results: Twenty-two percent of the dentists have reported at least one allergic patient reaction with more implication of fixed appliances. Eleven percent of adverse effects locations reported were mucosal area and facial skin near contact area and less than 5% was systemic eruption outside contact site. Only 34% of the respondents knew the Nickel allergy etiology.
Conclusion: Nickel-containing orthodontic alloys are generally used even in patient with suspected nickel allergy. It is very important to minimize corrosion factors and use nickel-free appliances in clinical practice.
