Résumé

L’anesthésie des dents atteintes de MIH (Molar and Incisor Hypomineralisation) est un des défis majeurs de notre pratique, notamment chez les enfants. L’anesthésie par injection intra-osseuse assistée par ordinateur (CAIO) d’une solution d’articaïne à 4% avec adrénaline à 1:200 000 (Alphacaine, Septodont™) a montré son efficacité chez l’enfant. Le but de ce travail a été d’évaluer son efficacité sur des dents atteintes de MIH. Cette étude préliminaire a été réalisée en utilisant le système Quick Sleeper™ (Dental Hi Tec™, Cholet, France) qui combine une rotation de l’aiguille pour pénétrer l’os et une injection de la solution anesthésique, le tout assisté par ordinateur. Dans cette étude ont été inclus 39 patients du département d’odontologie pédiatrique du CHU de Rennes pour 46 sessions (dont 32 sur premières molaires permanentes mandibulaires). CAIO a montré son efficacité dans 93.5% des cas (43/46). Les échecs observés (3) étaient dus à une impossibilité d’atteindre l’os spongieux (1) ou d’anesthésier complètement la dent (2). Cette étude préliminaire suggère que l’anesthésie intra-osseuse assistée par ordinateur est une technique prometteuse pour l’anesthésie des dents atteintes de MIH qui peut être utilisée en routine par les praticiens entraînés.

Abstract
Computer-assisted intraosseous anaesthesia for molar and incisor hypomineralisation teeth. A preliminary study

Anesthetizing MIH (Molar and Incisor Hypomineralisation) teeth is one of the major challenges in paediatric dentistry. Computer-assisted IO injection (CAIO) of 4% articaine with 1:200,000 epinephrine (Alphacaine, Septodont™) has been shown to be an efficient way to anesthetize teeth in children. The aim of this study was to assess the efficacy of this method with MIH teeth. This preliminary study was performed using the Quick Sleeper™ system (Dental Hi Tec™, Cholet, France) that allows computer-controlled rotation of the needle to penetrate the bone and computer-controlled injection of the anaesthetic solution. Patients (39) of the department of Paediatric Dentistry were included allowing 46 sessions (including 32 mandibular first permanent molars) to be assessed. CAIO showed efficacy in 93.5% (43/46) of cases. Failures (3) were due to impossibility to reach the spongy bone (1) and to achieve anaesthesia (2). This prospective study confirms that CAIO anaesthesia is a promising method to anesthetize teeth with MIH that could therefore be routinely used by trained practitioners.
