Résumé

Objectif : Evaluer la qualité des restaurations coronaires proximales et mettre en évidence les facteurs influençant cette qualité.
Matériel et méthode : Une étude transversale sur 160 clichés radiographiques de restaurations coronaires proximales a été réalisée au Centre Universitaire de Consultations et de Traitements Dentaires de Rabat. Les restaurations ont été évaluées selon les critères USPHS modifiés. Quatre variables ont été étudiées : le matériau de restauration, la dent traitée, le type de cavité et le lieu de traitement afin de mettre en évidence les facteurs influençant.
Résultats : Quarante-cinq pour cent des restaurations coronaires proximales évaluées ont nécessité un remplacement. La cause principale d’échec était la récidive carieuse dans 36% des cas, suivie de l’absence du point de contact 24,4%, un contour coronaire proximal inacceptable 13,8%, et enfin l’exposition dentinaire 6,9%. Ainsi, il s’est avéré qu’il existe une difficulté pour restaurer correctement les cavités proximales postérieures notamment à l’amalgame et un taux élevé de récidive carieuse après restauration aussi bien au niveau des dents antérieures que postérieures.
Conclusion : Une évaluation systématique aussi bien clinique que radiographique après chaque restauration proximale serait judicieuse afin de réduire l’échec.

Abstract
Evaluation of the quality of proximal restorations with plastic materials: radiographic study

Aim: To assess the quality of proximal coronary restorations and bring out the factors influencing this quality.
Material and method: A transversal study on 160 radiographs of proximal coronary restorations was conducted at the University Center of Dental Consultation and Treatment of Rabat. Restorations were evaluated according to the modified USPHS criteria. Four variables were studied: restoration material, treated tooth, cavity type and location of treatment in order to identify the influencing factors.
Results: Forty five per cent of the evaluated proximal coronary restorations required replacement. The main cause of failure was recurrent caries in 36% of cases, followed by the absence of the contact point (24.4%), unacceptable proximal coronary outline (13.8%), and finally dentin exposure (6.9%). Thus, it turned out that there is a difficulty to restore correctly posterior proximal cavities especially using amalgam and a high rate of recurrent caries at both the anterior and posterior teeth.
Conclusion: Both a clinical and a radiographic evaluation after each proximal restoration would be recommended to reduce failure.
