Résumé

Décrit pour la première fois par Philipsen en 1956, le kératokyste odontogénique est caractérisé par une importante kératinisation de sa bordure malpighienne, une croissance agressive et une forte propension aux récidives. Depuis 2005, il a été classé parmi les tumeurs odontogènes des maxillaires prenant ainsi le nom de Tumeur Odontogénique Kératokystique (TOK). Cliniquement, la TOK se manifeste par une tuméfaction le plus souvent asymptomatique. Radiologiquement, il s’agit d’une lésion ostéolytique uniloculaire ou multiloculaire.
L’approche diagnostique repose sur l’analyse conjointe des données de l’anamnèse, de la sémiologie clinique et du bilan radiologique.
La confirmation est apportée par l’examen anatomopathologique.
Enfin, le traitement est chirurgical et l’évolution est marquée par une forte propension à la récidive. Les auteurs rapportent le cas d’un patient porteur d’une tumeur kératokystique odontogène de la mandibule et font le point sur les différents aspects diagnostiques, thérapeutiques et évolutifs de ce type de lésion.

Abstract
Keratocystic odontogenic tumor: a case report

First described by Philipsen in 1956, the odontogenic keratocyst is characterized by a large squamous keratinization of its border, an aggressive growth and a high recurrent rate. It is now designated by the World Health Organization as a Keratocystic Odontogenic Tumor (KOT). Clinically, the KOT is manifested by an asymptomatic growth. Radiographically, it appears as a well-defined unilocular or multilocular osteolytic lesion. The diagnostic approach is based on a combined analysis of the medical history, the clinical appearance and the radiographic appearance. The diagnosis may be confirmed by the anatomical pathology report. Finally, treatment consists of surgical excision and follow up is characterized by a high rate of recurrence. The authors report a case of keratocystic odontogenic tumor of the mandible and review the various diagnoses, therapeutics and follow up aspects of this type of tumors.
