Résumé

La restauration en composite des cavités proximales sur les molaires temporaires est l’une des thérapeutiques les plus fréquemment réalisées en dentisterie pédiatrique. Cependant, les pédodontistes souhaitent disposer d’une alternative à la technique d’obturation incrémentielle classique. L’introduction sur le marché du composite fluide à placement en masse SDR® et l’extension de ses indications, en 2014, à la restauration des cavités "classe I" et "classe II" sur les dents temporaires, sans recouvrement occlusal, a été un atout en dentisterie pédiatrique. Afin d’évaluer l’efficacité du SDR® dans la restauration des cavités "classe II", 60 deuxièmes molaires temporaires ont été sélectionnées en paire chez 30 enfants âgés de 6 à 8 ans. Chaque dent de la paire a été restaurée en proximal, soit en masse par le composite "bulk-fill" SDR®, soit en incréments par le composite universel nano-céramique Ceram.X®. L’évaluation des restaurations s’est basée sur les critères cliniques du système USPHS modifié, les critères radiographiques définis par l’absence ou la présence d’hiatus sur la radiographie digitale et le temps opératoire spécifique à chaque méthode d’obturation. Des contrôles périodiques ont été effectués juste après placement de la restauration puis après 3, 6 et 9 mois. Pour comparer les variables entre les deux méthodes de restauration, les tests Exact de Fisher, les tests de Mac Nemar et les tests de Chi-deux de tendance ont été utilisés. Les résultats ont montré que le SDR® est plus rapide, plus facile à manipuler et plus performant que le Ceram.X® du point de vue adaptation marginale et absence de décoloration marginale. En ce qui concerne la teinte, le Ceram.X® est plus esthétique. Pour la rétention, les caries secondaires, l’état de surface, la forme anatomique, la sensibilité post-opératoire, le hiatus radiologique et le contact inter-proximal, il n’y avait pas de différence statistiquement significative entre les deux techniques d’obturation. Le SDR® est donc un matériau prometteur dans la restauration des dents temporaires, surtout au niveau des cavités "classe II".

Abstract
A randomized clinical trial of “class II” composite restorations in second temporary molars: A comparison between bulk filling and incremental filling

Composite restoration of "class II" cavities in primary molars is one of the most performed therapies in pediatric dentistry. In an attempt to expedite the conventional layering technique, new restorative “bulk-fill” composites such as the SDR® have been introduced to the market. In 2014, the SDR® indication was further extended on to the restoration of “class I” and “class II” cavities in primary molars without any additional capping, and this was a valuable asset in pediatric restorative dentistry. To test the efficiency of the SDR®, a clinical study was conducted on 30 healthy patients aged between 6 and 8 years old. In total, 60 second primary molars requiring proximal composite restorations were selected in pairs and restored by either the "bulk-fill" flowable composite SDR® or the universal nano-ceramic restorative composite Ceram.X®. All the restorations were evaluated clinically using slightly modified USPHS criteria and radiographically by the presence or absence of radiographic hiatus at baseline and periodically at 3, 6 and 9 months. The time needed to complete the obturation phase was measured and assessed for both types of composites. All the observations were compared using the Fisher’s exact test, the Mac Nemar test, and the Chi-square test X2. The SDR® was faster, easier to manipulate, and showed better marginal adaptation with less marginal discoloration than the Ceram.X®. The latter matched better adjacent tooth structure in color and translucency. As for the retention, the secondary caries, the surface texture, the anatomic form, the post-operative sensitivity, the presence of radiographic hiatus, and the quality of the interproximal contact, no significant differences were found between the two types of restorations. Therefore, the SDR® is a promising restorative material on primary molars, especially in restoring "class II" cavities.
