Résumé

Introduction : Dans le contexte africain, il n’est pas rare de voir en pratique courante des tumeurs maxillo-faciales très évoluées dont la prise en charge constitue un véritable défi. Le but du traitement de ces tumeurs est non seulement de traiter la tumeur, obtenir un résultat fonctionnel et esthétique optimal et éviter la récidive. L’objectif de ce travail était d’étudier le profil épidémio-clinique, anatomopathologique et thérapeutique des tumeurs maxillo-faciales géantes.
Patients et méthodes : Il s’est agi d’une étude transversale descriptive réalisée de 2017 à 2022 qui a concerné tous les cas de tumeurs géantes maxillo-faciales opérés dans le service. Les variables étudiées ont été compilées et analysées avec le logiciel SPSS 2.0.
Résultats : L’échantillon était composé de 20 cas dont 12 hommes (60%). L’âge moyen des patients était de 35 ans ± 1,6. Le délai moyen de consultation était de 6 ans avec des extrêmes respectivement de 3 ans et 15 ans. La tuméfaction maxillo-faciale était le principal motif de consultation (100%). La mandibule était concernée dans 14 cas (70%). L’examen anatomo-pathologique a révélé 19 cas (95%) de tumeurs bénignes dont 9 cas (45%) d’améloblastome. Le traitement a été essentiellement chirurgical (100%).
Discussion : Le traitement de ces volumineuses tumeurs est complexe. Lorsqu’il est encore possible d’intervenir, les choix thérapeutiques sont souvent limités au traitement radical avec ou sans reconstruction. La reconstruction permet une restitution fonctionnelle et esthétique.
Conclusion : Le traitement des tumeurs maxillo-faciales géantes nécessite un renforcement des capacités en ressources humaines et matérielles des structures.

Abstract
Management of giant maxillofacial tumours at the Odontostomatology National Centre Professor Hamady Traore in Bamako, Mali

Introduction: In the African context, it is not uncommon to see highly advanced maxillofacial tumours in current practice, and their management is a real challenge. The aim of treating these tumours is not only to treat the tumour, but also to obtain an optimal functional and aesthetic result and avoid recurrence. The aim of this study was to investigate the epidemioclinical, anatomopathological and therapeutic profile of giant maxillofacial tumours.
Patients and methods: This was a descriptive cross-sectional study conducted at the Professor Hamady Traoré National Center for Odonto-Stomatology in Bamako from 2017 to 2022. It included all cases of giant maxillofacial tumors diagnosed histologically and undergoing surgical treatment. The variables studied were compiled and analyzed using SPSS 2.0 software.
Results: The sample consisted of 20 cases, 15 of which were men (60%). The mean age of the patients was 35 ± 1.6 years. The average time to consultation was 6 years, with extremes of 3 and 15 years respectively. Maxillofacial swelling was the main reason for consultation (100%). The mandible was involved in 14 cases (70%). Anatomopathological examination revealed 19 cases (95%) of benign tumours, including 9 cases (45%) of ameloblastoma. Treatment was mainly surgical (100%).
Conclusion: Maxillofacial tumor pathology is diverse and varied. Its management implies strengthening the human and material resources capacities of health structures for better quality care for all.
