Résumé

Introduction : Pendant le traitement orthodontique, le praticien est souvent amené à faire des extractions dentaires ; ce qui pose la question de leurs effets sur les largeurs intra-arcades, le profil, la durée du traitement, les résultats occlusaux, l’esthétique du sourire et la stabilité. L’objectif de ce travail était de comparer les dimensions transversales des arcades maxillaires et mandibulaires avant et après traitement avec ou sans extraction dentaire.
Matériel et méthodes : Nous avons analysé les dossiers de 60 patients, 30 cas traités avec extractions des 1ères prémolaires et 30 cas traités sans extraction. La sélection de ces cas respecte les critères d’inclusions suivantes : une denture permanente complète avant traitement ; cas terminés et débagués en respectant les objectifs thérapeutiques. Pour les cas traités sans extractions, l’encombrement ne dépassait pas 5 mm. Nous avons mesuré à l’aide d’un pied à coulisse mécanique pour chaque patient la Distance Intercanine Maxillaire (DICS) et la Distance Intercanine Mandibulaire (DICI), la distance intermolaire maxillaire (DIMS) et la Distance Intermolaire Mandibulaire (DIMI) avant et après traitement. Les comparaisons statistiques ont été réalisées à l’aide d’un logiciel de statistiques SPSS (version 18), en adoptant un seuil de significativité à p < 0,05.
Résultats : Nous avons trouvé dans le groupe avec extraction une augmentation significative de la DICS de 1,6 mm (p < 0,05) ; alors que la DICI a subi une légère augmentation non-significative (p > 0,05). Une diminution significative de la DIMS de 2,2 mm (p < 0,05) ; alors que la DIMI a subi une légère diminution non-significative (p > 0,05). Dans le groupe sans extraction, nous avons trouvé une augmentation significative de toutes les mesures du sens transversal après traitement.
Conclusion : Les modifications de la forme d’arcade avec le traitement avec extraction et surtout sans extraction sont réelles et significatives. Ces modifications doivent être prises en compte pendant le traitement afin de garantir une stabilité thérapeutique à moyen et à long terme.

Abstract
Changes in the transverse dimensions of the dental arch after orthodontic treatment with and without extraction of premolars

Introduction: During orthodontic treatment, the practitioner is often required to perform dental extractions; this raises the question of their effects on intra-arch widths, profile, treatment duration, occlusal results, smile aesthetics and stability. The objective of this work is to compare the transverse dimensions of the maxillary and mandibular arches before and after treatment with or without dental extraction.
Material and methods: We analyzed the files of 60 patients, 30 cases treated with extraction of the 1st premolars and 30 cases treated without extraction. The selection of these cases met the following inclusion criteria: a complete set of permanent teeth before treatment; cases completed and degumming while meeting the therapeutic objectives. For cases treated without extractions, crowding did not exceed 5 mm. We measured the maxillary intercanine distance (ICD) and mandibular intercanine distance (MICD), maxillary intermolar distance (MIID) and mandibular intermolar distance (MIID) for each patient before and after treatment using a mechanical caliper. Statistical comparisons were performed using SPSS statistical software (version 18), adopting a significance threshold of p < 0.05.
Results: We found a significant increase in ICD of 1.6 mm in the extraction group (p < 0.05), while the ICD underwent a slight, non-significant increase (p > 0.05). A significant decrease in MIID of 2.2 mm (p < 0.05), while the MIID underwent a slight, non-significant decrease (p > 0.05). In the non-extraction group, we found a significant increase in all transverse measurements after treatment.
Conclusion: Arch shape changes with extraction and especially non-extraction treatment are real and significant. These changes must be taken into account during treatment to ensure medium- and long-term therapeutic stability.
