Résumé

Contexte : Le dossier médical patient contribue à la continuité, la sécurité et à l’efficacité des soins. En odonto-stomatologie, il permet de situer les responsabilités en cas de litige mais aussi d’identifier une personne. Cependant, dans de nombreux centres de soins, ce dossier est inexistant ou inexploitable. L’objectif était de faire un état des lieux de la gestion du dossier patient dans les structures de soins dentaires en Côte d’Ivoire.
Méthodes : Une étude transversale a été conduite auprès des chirurgiens-dentistes, lors d’une formation continue en 2023. Un questionnaire a permis de recueillir les caractéristiques sociodémographiques et des informations sur la gestion du dossier patient (documents utilisés pour consigner les données, éléments constitutifs du dossier patient). Les variables ont été décrits par des effectifs, des fréquences et par des médianes avec leur Intervalle Interquartile (IIQ). La comparaison des fréquences a été faite avec le test du Chi2 de Pearson.
Résultats : Quatre-vingt-quatre chirurgiens-dentistes, d’âge médian de 49 ans (IIQ : 42-56), ont été enquêtés. Pour 75 (89,3%) des chirurgiens-dentistes, la fiche de soin dentaire était le document principal de consignation des données. Parmi ceux-ci, 68 (81,0%) considéraient que cette fiche constituait le seul élément du dossier patient. La totalité des praticiens du privé mettait à jour le schéma dentaire par rapport à ceux du public 12 (100%) vs 52 (75,4%) p = 0,053. Quatre-vingt-trois (98,8%) ignoraient les autres éléments constitutifs du dossier patient. En effet, 25 (32,5%) ignoraient qu’un certificat médical établi faisait partie du dossier patient.
Conclusion : Cette enquête a montré que la méconnaissance du dossier patient est une réalité. Un renforcement des capacités des chirurgiens-dentistes pour la constitution et la bonne tenue du dossier patient reste une priorité pour une prise en charge de qualité et un meilleur suivi des patients.

Abstract
Management of patient’s medical records in dental care facilities in Ivory Coast

Background: Patients’ medical records contribute to the continuity, safety and effectiveness of care. In odonto-stomatology, they help to identify individuals and to establish responsibility in the event of a patient-physician dispute. However, in many health care facilities, these records don’t exist or are unusable. The aim was to take stock of patient file management in dental care facilities in Ivory Coast.
Methods: A cross-sectional study was conducted among dental surgeons during a continuing education course in 2023 in Abidjan. A questionnaire was used to collect socio-demographic characteristics and information on patient record management (documents used to record data, components of the patient record). The variables were described in terms of numbers, frequencies and medians with their Interquartile Ranges (IQR). Frequencies were compared using Pearson’s Chi-square test.
Results: A total of 84 dentists, with a median age of 49 (IQR: 42-56), were surveyed. For 75 (89.3%) of the dentists, the dental care record form was the main document for recording data. Of these, 68 (81.0%) considered that this form was the only element of the patient record. All of the private practitioners updated the dental chart compared to those in the public sector 12 (100%) vs 52 (75.4%) p = 0.053. Eighty-three (98.8%) were unaware of the other components of the patient record. In fact, 25 (32.5%) were unaware that a medical certificate was part of the patient file.
Conclusion: This survey showed that patient records are notoriously unfamiliar. Building the capacity of dentists to create and properly maintain patient records remains a priority for quality care and improved patient follow-up.
