L'iléite tuberculeuse est la localisation préférentielle de la tuberculose intestinale et peut se confondre avec une maladie de Crohn iléale. Nous rapportons le cas d'un patient hospitalisé pour une iléite tuberculeuse révélée par un syndrome de Koenig et une altération de l'état général. Il avait été suivi et traité comme une maladie de Crohn depuis le mois de novembre 2012, sans amélioration de l'état clinique. Le scanner abdominal avait montré un épaississement de la dernière anse iléale avec quelques adénopathies calcifiées. La coloscopie était normale mais l'iléon n'avait pu être cathétérisé. La biopsie par voies chirurgicales des adénopathies concluait à une tuberculose. Un traitement spécifique avait été débuté, améliorant l'état général du patient. Devant une iléite terminale, il ne faut pas oublier l'origine tuberculeuse.
Tubercular ileitis is the main manifestation of intestinal tuberculosis and often confused with ileal Crohn's disease. We report the case of a patient with hospitalized for tubercular ileitis revealed by a Koenig syndrome and impairment of general condition. He was initially treated as a Crohn's disease since November 2012 without clinical improvement. Abdominal CTscan revealed a wall thickening of the terminal ileum with some calcified nodes. The colonoscopy was normal and the ileonoscopy was impossible. Histopathological analysis of the node revealed the tuberculosis. The treatment contributed to the amelioration of the health's patient. In conclusion, tuberculosis ileitis must be search when there is a terminal ileitis.
