L'hémothorax post-traumatique est une urgence médico-chirurgicale dont la fréquence sans cesse croissante, touche le plus souvent les adolescents.
Le but de ce travail était d'étudier les aspects épidémiologiques cliniques et thérapeutiques de cette pathologie.
Il s'agissait d'une étude rétrospective de 42 mois portant sur 37 dossiers de patients admis pour un hémothorax post-traumatique dans le service de chirurgie thoracique de l'Hôpital National Donka en République de Guinée.
Nous avons colligé 37 dossiers de patients après application de nos critères de sélection. La fréquence hospitalière de l'hémothorax était de 9,25%.
L'âge moyen était de 32,88 ans et un sex-ratio de 8,25. Les accidents de la voie publique étaient les principaux pourvoyeurs d'hémothorax. L'hémothorax était de grande abondance chez 21 patients (56,76%), de moyenne abondance chez 15 patients (40,54%) et minime chez un seul patient (2,7%).
Quatre-vingt-dix-sept virgule trente pour cent (97,30%) de nos patients ont eu une amélioration clinique et radiologique immédiate après le drainage pleural. Parmi ces patients, un seul a présenté une complication à type d'infection chez qui, la thoracotomie de décortication pulmonaire a été pratiquée pour non-retour du poumon à la paroi thoracique après un drainage prolongé. Nous avons enregistré un cas de décès (2,70%).
The post-traumatic hemothorax is a surgical emergency that mid-frequency growing, most often affects adolescents.
The aim of this study was to investigate the clinical and therapeutic epidemiological aspects of this disease.
This was a retrospective study of 42 months on 37 cases of patients admitted for post-traumatic hemothorax in the service of Thoracic Surgery Donka National Hospital in the Republic of Guinea.
We collected 37 cases of patients after application of our selection criteria. Hospital frequency of hemothorax was 9.25%.
The mean age was 32.88 years and a sex ratio of 8.25. Accidents of highways were the main outfitters hemothorax. The hemothorax was abundance in 21 patients (56.76%), mean abundance in 15 patients (40.54%), and minimal in one patient (2.7%). 97.30% of our patients had an immediate clinical and radiological improvement after pleural drainage. Among these, one patient had presented a pleuritis encysted, in which thoracotomy lung decortication was convenient for non-return lung to the chest wall after prolonged drainage. We have registered a case of death (2.70%).
