Objectif : Déterminer les principales complications et les causes de la cirrhose à Bangui.
Patients et méthode : Il s’agissait d’une étude prospective descriptive d’une durée de 20 mois menée dans le service d’hépato-gastroentérologie et de médecine interne de l’Hôpital Universitaire de l’Amitié de Bangui. Etaient inclus dans l’étude, tous les patients âgés d’au moins 18 ans chez lesquels le diagnostic de cirrhose décompensée était retenu quelle que soit l’étiologie.
Résultats : Neuf-cent soixante-trois (963) patients étaient hospitalisés pendant la période d’étude. Le diagnostic de cirrhose était retenu chez 190 patients (19,7%). La cirrhose était décompensée dans 184 cas (19,1%). Il s’agissait de 122 hommes (65,9%) et 62 femmes (34,1%). Le sex-ratio (H/F) était de 1,97. L’âge moyen de nos patients était de 44 ans avec des extrêmes de 18 et 88 ans. Les principales complications de la cirrhose étaient l’ascite associée ou/non à l’œdème des membres inférieurs (74,9%), l’hémorragie digestive (35%), l’infection du liquide d’ascite (23,3%), l’encéphalopathie hépatique (22,3%), le carcinome hépatocellulaire (20,2%), le syndrome hépatorénal (2,2%). L’étiologie de la cirrhose était post-hépatitique B (62,5%), post-hépatitique B et alcoolique (3,3%), alcoolique (17,9%), post-hépatitique C (8,7%), post-hépatitique B et D (3,8%), et indéterminée (3,8%).
Conclusion : La cirrhose est découverte souvent au cours des complications. La présence d’un facteur de risque de cirrhose doit amener à poser précocement le diagnostic dont la prise en charge permettrait d’éviter ou de retarder la survenue des complications.
Objective: To determine the main complications and the reasons of the cirrhosis in Bangui.
Patients and method: It was about a descriptive prospective survey with a duration of 20 months in the hepatology gastroenterology and internal medicine units of the University hospital of “Amitié” of Bangui. Were included in the survey, all patients aged at least 18 years in whom the diagnosis of complicated cirrhosis was retained regardless of the etiology.
Results: Nine hundred and sixty-three (963) patients were hospitalized during study period. The diagnosis of cirrhosis was retained in 190 patients (19,7%). The cirrhosis was decompensated in 184 cases (19,1%). It was about 122 (65,9%) men and 62 (34,1%) women. The sex-ratio (H/F) was 1,97 in favor of men. The average age of our patients was of 44 years with extremes of 18 and 88 years. The main complications of the cirrhosis were ascites associated or/no with edema of the lower limbs (74,9%), digestive hemorrhage (35%), infection of ascite fluids (23,3%), hepatic encephalopathy (22,3%), hepatocellular carcinoma (20,2%), hepatorenal syndrome (2,2%). The etiology of cirrhosis was post hepatitic B (62,5%), post hepatitic B and alcoholic (3,3%), alcoholic (17,9%), post hepatitic C (8,7%), post hepatitic B and D (3,8%), and indeterminate (3,8%).
Conclusion: Cirrhosis is often discovered during complications. The presence of a risk factor for cirrhosis should lead to early diagnosis and the management would permit to avoid or delay the onset of complications.
