But : Rapporter les lésions œsogastroduodénales retrouvées à la fibroscopie digestive haute et la fréquence d’Helicobacter pylori chez le cirrhotique.
Patients et méthode : Etude rétrospective menée durant une année sur les dossiers des patients hospitalisés dans le service d’hépato-gastro-entérologie du CHU Campus de Lomé pour une cirrhose quelle que soit son étiologie et ayant bénéficié d’une fibroscopie digestive haute.
Résultats : Notre échantillon comportait 103 patients avec une prédominance masculine (sex-ratio : 1,78) ; l’âge moyen était de 50,15 ans (extrêmes : 29 et 79 ans). Soixante-dix-sept patients (74,75%) prenaient un traitement traditionnel. La principale indication de la fibroscopie digestive haute était les épigastralgies dans 38 cas (37,86%). L’augmentation du volume abdominal constituait le motif de consultation le plus fréquent chez 72 patients (69,90%). On a objectivé des signes d’hypertension portale chez 45 patients (43,69%) ; les gastropathies fundique et antrale, les bulbites ulcérative et érythémateuse ont été retrouvées respectivement chez 13 patients (12,62%), 11 patients (10,68%), 7 patients (6,79%) et 2 patients (1,94%). Il y avait 16 cas d’ulcère gastrique et 18 cas d’ulcère bulbaire. Seuls 22 patients ont pu honorer l’examen anatomopathologique des fragments biopsiés et la recherche d’Helicobacter pylori était positive chez 16 patients (72,73%).
Conclusion : La pathologie inflammatoire et la maladie ulcéreuse gastroduodénales sont fréquentes chez le cirrhotique. Les facteurs associés sont Helicobacter pylori et le traitement traditionnel.
Purpose: To report the lesions found during upper digestive endoscopy and the frequency of Helicobacter pylori in cirrhotic patients.
Patients and methods: Retrospective study conducted during one year on the records of patients hospitalized in the department of hepato-gastroenterology of the University Hospital Campus of Lome for cirrhosis regardless its etiology and who benefited from an upper gastrointestinal endoscopy.
Results: Our sample included 103 patients with a male predominance (sex ratio: 1.78); the average age was 50.15 years old (extremes: 29-79 year-old). Seventy-seven patients (74.75%) took a traditional treatment. The main indication for upper gastrointestinal endoscopy was the epigastric pain in 38 cases (37,86%). The increase of abdominal volume was the most frequent reason for consultation in 72 patients (69,90%). We objectified signs of portal hypertension in 45 patients (43.69%); fundic and antral gastropathies, ulcerative and erythematous bulbites were found respectively in 13 patients (12.62%), 11 patients (10.68%), 7 patients (6.79%) and 2 patients (1.94%). There were 16 cases of gastric ulcer and 18 cases of bulbar ulcers. Only 22 patients have been able to honor the histological examination of biopsy fragments and the search for Helicobacter pylori was positive in 16 patients (72.73%).
Conclusion: Inflammatory pathology and gastroduodenal ulcer disease are frequent in the cirrhotic. Associated factors are Helicobacter pylori and traditional treatment.
