But : Déterminer le profil des lésions œso-gastroduodénales retrouvées en endoscopie digestive haute et relever les particularités.
Patients et méthode : Etude rétrospective, descriptive sur des résultats de compte-rendu d’endoscopie digestive haute menée en unité d’endoscopie digestive du service d’hépato-gastro-entérologie du CHU Campus de Lomé du 15 mai 2009 au 31 décembre 2013 chez des patients âgés de moins de 20 ans.
Résultats : Au cours de la période d’étude, 2796 fibroscopies digestives hautes ont été réalisées dont 201 chez des sujets âgés de moins de 20 ans (7,19%) ; il y avait une prédominance féminine (sex-ratio : 2,14). L’âge moyen était de 16,77 ans avec des extrêmes de 5 et 19 ans. La fibroscopie digestive haute était normale chez 59 patients (29,35%). La fibroscopie digestive haute avait été réalisée dans 137 cas (68,15%) pour épigastralgies. Nous avons noté 195 lésions endoscopiques. La pathologie gastrique 72,31%. La pathologie inflammatoire (œsophagite, gastrite, bulbo-duodénite) prédominait avec 54,86%, la pathologie ulcéreuse représentait 9,23%. On ne notait pas d’ulcère gastrique avant l’âge de 12 ans chez la fille et 17 ans chez le garçon. On ne retrouvait pas l’ulcère bulbaire avant l’âge de 14 ans chez la fille et 16 ans chez les garçons.
Conclusion : La fibroscopie digestive haute n’est pas de pratique courante chez les moins de 15 ans ; les lésions inflammatoires dominent les pathologies digestives hautes rencontrées chez les enfants et adolescents au Togo.
Aim: To determine the profile of upper gastrointestinal lesions found in the upper gastrointestinal endoscopy and identify features.
Method and patients: Retrospective study, descriptive on results of upper gastrointestinal endoscopy report carried out by the endoscopy unit of hepato-gastroenterology of the Teaching Hospital Campus of Lomé from May, the 15th 2009 to December, the 31st 2013 with patients under 20 years old.
Results: During the study period, 2796 upper gastrointestinal fiberscopies were performed with 201 under the age of 20 years old patients (7.19%); there was a female predominance (sex ratio: 2.14). The average age was 16.77 years old ranging from 5 to 19 year-old. There were 33 patients under the age of 15 year-old (16.42%). The upper gastrointestinal fiberscope was normal with 59 patients (29.35%). The upper gastrointestinal endoscopy was performed in 137 cases (68.15%) for epigastric pain. We noted 195 endoscopic lesions with a predominance of lesions in the age from 15 to 19 year-old. Gastric pathology represented 72.31%. Inflammatory disease (esophagitis, gastritis, bulbo-duodenitis) predominated with 54.86%, ulcerative disease showed 9.23%. We did not note gastric ulcer for boy before the age of 17 and girl before the age of 12. We did not find duodenal ulcer before the age of 14 years old for girls and 16 years old for boys.
Conclusion: The upper gastrointestinal endoscopy is not common practice among less than 15 year-old; inflammatory lesions dominate the upper gastrointestinal pathologies in children and teenagers in Togo.
