Il s’agit d’une patiente de 62 ans, admise pour dyspnée d’effort avec notion de toux nocturne, de vomissements post-prandiaux d'évolution progressive depuis plusieurs années. Pas d’antécédents particuliers mis à part une dysphagie intermittente, parfois paradoxale, et qui cédait lors de manœuvres posturales. Cette dysphagie évoluerait depuis 20 ans et était associée à des régurgitations et des douleurs thoraciques. La patiente présentait une AEG, avec un syndrome d’épanchement pleural liquidien, de même qu’une radiographie thoracique compatible. L’examen cytobactériologique du liquide présumé pleural avait retrouvé, un liquide exsudatif légèrement lymphocytaire. Le scanner thoracique réalisé dans un premier temps mettait en évidence une formation liquidienne pseudo-kystique du poumon droit. L’incertitude diagnostique motive une reprise de la TDM thoracique avec préparation, par ingestion de gastrographine® par la patiente. Cette TDM mettait en évidence une dilatation de l’œsophage thoracique, rempli de liquide et basculé sur l’hémi thorax droit, et a permis de conclure à un méga-œsophage thoracique simulant une pleurésie.
This is a patient of 62 years, admitted for exertional dyspnea with nocturnal cough and notion of early post prandial vomiting gradually evolving for several years. No particular medical past history apart an intermittent dysphagia, sometimes paradoxical and disappearing during postural maneuvers. This dysphagia evolve for 20 years and was associated with regurgitation and chest pain. The patient had a bad general aspect, with a syndrome of pleural fluid effusion, with a compatible chest radiograph. Cytobacteriological examination of pleural fluid had found an exudative fluid, slightly lymphocytic. CT scan, performed initially, highlighted a pseudo-cystic liquid formation of the right lung. The uncertainty of diagnosis motivates a resumption of chest CT with preparation of gastrograffin® swallowed by the patient. The CT showed an expansion of the thoracic esophagus, filled with liquid and switched to the right hemi thorax and allowed to conclude a chest mega-esophagus simulating pleurisy.
