Introduction : Le diagnostic du paludisme au Centre de Prévention et de Traitement du Paludisme (CPTP) de l'Hôpital de l'Amitié à Bangui en République Centrafricaine est posé sur la base de deux tests dont un Test de Dépistage Rapide (TDR) et une Goutte Epaisse (GE) accompagnée d'un frottis mince. Les TDR marqués CareStartTMMalaria HRP2 (Pf)®, SD Bioline Malaria Antigen Pf® et le Paracheck (Pf)® détectant tous la protéine riche en histidine (HRP2) de Plasmodium falciparum (P. falciparum) sont les plus utilisés au CPTP. L'objectif de cette étude était de comparer la performance de ces trois TDR du point de vue sensibilité, spécificité et de comparer les résultats avec ceux de la GE.
Matériels et méthodes : Cette étude transversale était réalisée au CPTP entre juillet et septembre 2016. Les patients d'âge supérieur ou égal à 6 mois à qui une GE est demandée étaient inclus dans l'étude. Pour chaque patient un frottis mince était réalisé pour identifier l'espèce plasmodiale. Nous avons cherché à établir une relation entre le temps de positivité du TDR et la densité parasitaire (DP). Le consentement de chaque patient était obtenu avant le prélèvement sanguin pour les analyses. Cette étude a bénéficié de l'autorisation du Ministère de la Santé.
Résultats : Au total, 500 patients âgés de 6 mois à 90 ans étaient inclus dans l'étude. Les tests positifs étaient de 273 dont 245 (89,74%) pour P. falciparum et 28 (10,26%) pour Plasmodium malariae (P. malariae). La sensibilité des trois TDR était inférieure à celle donnée par les fabricants. La spécificité des trois TDR était presque similaire à celle du fabricant. Pour une faible DP la barre de positivité du test apparaissait souvent après celle du contrôle. Il existe une différence significative entre la positivité de la GE et celle de CareStart et SD Bioline (p < 0,05).
Conclusion : Le TDR Paracheck (Pf)® est plus performant que les deux autres tests et doit être conseillé en Centrafrique. La GE et le frottis sanguin doivent être recommandés en cas d'utilisation d'un TDR détectant une seule espèce de Plasmodium.
Introduction: The diagnosis of malaria at the Center for Prevention and Treatment of Malaria of (CPTP) the Friendship Hospital in Bangui, Central African Republic is based on two tests including a Rapid Screening Test (RST) and a Thick Drop (TD) with a thin smear. The TDRs labeled CareStartTM Malaria HRP2 (Pf)®, SD Bioline Malaria Antigen Pf® and Paracheck (Pf)® all detect the histidine-rich protein (HRP2) of Plasmodium falciparum (P. falciparum). The objective of this study was to compare the performance of these three TDRs from the point of view of sensitivity, specificity and to compare the results with those of the TD.
Materials and methods: This cross-sectional study was carried out at the CPTP between July and September 2016. The patients of six months or more of age, without sex, and with EGs was included in the study. For each patient a thin smear was performed to identify other Plasmodium species that may interfere with the comparison of results. We sought to establish a relationship between the positivity time of TDR and Parasite Density (PD). The consent of each patient was obtained before blood sampling for the analyzes. This study was approved by the Ministry of Health.
Results: A total of 500 patients aged 6 months to 90 years were included in the study. Positive tests were 273 of which 245 (89.74%) were for P. falciparum and 28 (10.26%) for Plasmodium malariae (P. malariae). The sensitivity of the three RDTs was lower than that of the manufacturers. The specificity of the three TDRs was almost similar to that of the manufacturer. For a low PD the positivity bar of the test often appeared after that of the control. There was a significant difference between the positivity of GE and that of CareStart and SD Bioline (p < 0.05).
Conclusion: The Paracheck (Pf)® TDR is better performing than the other two tests and should be advised in the Central African Republic. EG and blood smear should be recommended when using a TDR that detects a single species of Plasmodium.
