Résumé

Introduction : Les tumeurs intracrâniennes demeurent un problème de santé publique avec une incidence estimée à 1,6% et un taux de mortalité de 2,5% dans le monde en 2020. L’objectif de notre étude était d’évaluer la performance du scanner cérébral par rapport à l’anatomopathologie dans le diagnostic des tumeurs intracrâniennes au Mali.
Matériels et méthode : Il s’agissait d’une étude transversale allant du 1er août 2022 au 31 juillet 2023. Elle a concerné 60 patients ayant bénéficiés d’un examen de tomodensitométrie cérébrale et histo-anatomopathologie. L’échantillonnage était de type aléatoire. Les analyses de données ont été effectuées à l’aide du logiciel SPSS version 25. Le coefficient de concordance Kappa de Cohen a été calculé. L’anonymat et la confidentialité ont été respectés.
Résultats : L’âge moyen était de 45 ± 2 ans et 55% étaient des hommes. La clinique était dominée par les céphalées (68,3%) et le flou visuel (50%). Une concordance parfaite a été trouvé entre les deux examens dans le diagnostic du craniopharyngiome avec un coefficient de concordance (K = 100%), celui d’adénome hypophysaire était 79,2%, un coefficient de 71,9% a été trouvé pour le méningiome suivi de l’astrocytome (65,9%).
Conclusion : Nos résultats démontrent une bonne concordance entre la tomodensitométrie et l’examen anatomo-histopathologique montrant l’important de la TDM dans le diagnostic des tumeurs intracrâniennes.

Performance of cerebral CT compared with anatomopathology in the diagnosis of intracranial tumours in Mali: a case report of 60 patients

Introduction: Intracranial tumours remain a public health problem, with an estimated incidence of 1.6% and a mortality rate of 2.5% worldwide in 2020. The aim of our study was to evaluate the performance of brain scans compared with anatomopathology in the diagnosis of intracranial tumours in Mali.
Materials and method: This was a cross-sectional study from August 1st, 2022, to July 31st, 2023. It included 60 patients who had undergone a cerebral CT scan and histo-anatomopathology. Sampling was random. Data analysis was performed using SPSS version 25 software. Cohen’s kappa coefficient of agreement was calculated. Anonymity and confidentiality were assured.
Results: The mean age was 45 ± 2 years and 55% were men. Clinical features were dominated by headache (61.7%) and blurred vision (50%). A perfect concordance was found between the two examinations in the diagnosis of craniopharyngioma with a concordance coefficient (K = 100%), that of pituitary adenoma was 79.2%, a coefficient of 71.9% was found for meningioma followed by astrocytoma (65.9%).
Conclusion: Our results show a good concordance between CT and anatomo-pathological examination, demonstrating the importance of CT in the diagnosis of intracranial tumors.
