Nous avons réalisé une étude sur les oedèmes des membres inférieurs (OMI) recensés dans le service de Médecine Interne du CHU de Treichville.
Objectif : Préciser la prévalence des OMI et en rapporter les étiologies.
Patients et méthode : Il s’agit d’une étude prospective portant sur 339 malades hospitalisés et/ou suivis pour OMI dans ledit service du 1er janvier 2004 au 30 juin 2005.
Résultats : La prévalence des OMI s’établit à 6,3 %. L’âge moyen des patients était de 44 ans avec des extrêmes de 15 ans et 90 ans. Le tableau clinique était essentiellement celui de l’insuffisance cardiaque (35,6 %), du syndrome oedémato-ascitique (29,9 %), l’oedème bilatéral isolé (15,3 %) et l’anasarque (14,7 %). Au plan étiologique, les principaux organes atteints étaient le coeur (44 %), le foie (26 %) et le rein (18.3 %). L’atteinte cardiaque était dominée par la myocardiopathie d’origine hypertensive (45,2 %). L’HTA pulmonaire était en majorité associée à l’infection à VIH (4 cas sur 6). L’oedème hépatique était surtout dû à la cirrhose post-hépatitique décompensée (63,4 %). Le syndrome néphrotique (67,3 %) secondaire à l’infection à VIH dominait les causes des oedèmes rénaux. La dénutrition (32,7 %), l’érysipèle (26,9 %) et la phlébite (17,3 %) étaient les principales étiologies des oedèmes localisés. La dénutrition était surtout due à l’infection à VIH (52,6 %) et au cancer (26,3 %).
Conclusion : La myocardiopathie, la cirrhose, le syndrome néphrotique et la dénutrition sont les principales causes des OMI.
We realized a study on the edemas of lower limbs (OLL) listed in the service of internal medicine of the center of Treichville.
Objective : To precise the prevalence of the OLL and report the etiologies.
Patients and method : It’s a prospective study concerning 339 patients hospitalized and followed in the over- mentioned service from January 1st, 2004 to June 30th 2005.
Results : The prevalence is established in 6.3%. The average age was 44 years. The clinical picture was essentially that of cardiac insufficiency 35.6%, edemato-ascitic syndrome 29.9%, bilateral isolated edema 15.3% and anasarca 14.7%. On the etiological plan, the main affected organs were the heart (44%), the liver (26%) and the kidney (18.3%). The cardiac affection was dominated by the myocardiopathy of hypertensive origin. The pulmonary HTA was most of the time associated to HIV affection (4 cases on 6). The hepatic edema was especially due to post-hepatitic cirrhosis. The nephrotic syndrome (67.3%) secondary to HIV affection was the main cause of renal edemas. The undernutrition (32.7%), the erysipela (26.9%) and the phlebitis (17.3%) were the etiologies of the localized edemas. The undernutrition was especially due to the HIV affection and to cancer.
Conclusion : The myocardiopathy, the cirrhosis, the nephrotic syndrome and undernutrition are the main reasons of the edemas of lower limbs.
