La tuberculose est une maladie contagieuse à transmission essentiellement interhumaine due aux complexes Mycobacterium tuberculosis. La tuberculose des séreuses est définie comme une inflammation des séreuses due au bacille de Koch. Nous avions réalisé une étude rétrospective et descriptive chez les enfants hospitalisés à l’Hôpital d’Enfant Albert Royer du 1er juin 2021 au 30 juin 2022. Ont été inclus tous les enfants âgés de 2 mois à 15 ans hospitalisés pour une tuberculose impliquant au moins une atteinte séreuse. Les données ont été recueillies à partir des dossiers d’hospitalisation et saisies puis analysés sur Excel 2010. Durant la période d’étude, 188 patients ont été hospitalisés pour une tuberculose et 25 avait une forme séreuse soit une incidence de 13,8%, le sex-ratio de 2,1 et l’âge moyen de 9 ans. Une malnutrition sévère était retrouvée chez 10 patients soit 40,0%. Vingt patients étaient bien vaccinés au BCG. Une notion de contage tuberculeux était retrouvée chez 7 patients (28,0%). Le syndrome d’épanchement pleural, une ascite, un syndrome d’épanchement péricardique étaient retrouvé respectivement dans 68,0%, 20,0% et 12,0%. Le liquide de ponction était macroscopiquement jaune citrin dans 96,0% des cas. L’intradermoréaction à la tuberculine (IDRT) a été réalisée chez 4 patients (16.0%) avec un taux de positivité de 100,0% contre 63,7% pour le GeneXpert et les ADAs (adénosine désaminase) réalisées chez 7 patients (28.0%). Dix-sept patients avaient une tuberculose dans ses formes pleurales (68,0%), 10 formes péritonéales (40,0%) et 4 formes péricardiques (16,0%). Nous avons eu 18 cas de tuberculose multifocale (72,0%). Le protocole RHZE était utilisé chez tous les patients pendant 2 mois puis le protocole RH pendant 4 mois. Trois patients (12,0%) ont bénéficié d’une corticothérapie. Un cas de décès a été recensé (4.0%).

Serous tuberculosis: epidemiological, diagnostic and evolutive aspects at the Albert Royer Hospital in Dakar, Senegal

Tuberculosis is a contagious disease with essentially human-to-human transmission due to Mycobacterium tuberculosis complex. Tuberculosis of the serous membranes is defined as an inflammation of the serous membranes due to the bacillus of Koch. We carried out a retrospective and descriptive study in children hospitalized at Albert Royer Children’s Hospital from June 1st, 2021, to June 30th, 2022. All children aged 2 months to 15 years hospitalized for tuberculosis included a serous involvement. Data were collected from hospital records and entered and then analyzed on Excel 2010. During the study period, 188 patients were hospitalized for tuberculosis and 25 had a serous form, an incidence of 13.8%. The sex ratio was 2.1 and the average age was 9 years. Severe malnutrition was found in 10 patients, i.e. 40.0%. Twenty patients were well vaccinated with BCG. A notion of tuberculosis contamination was found in 7 patients (28.0%). Pleural effusion syndrome, ascites and pericardial effusion syndrome were found respectively in 68.0%, 20.0% and 12.0%. The puncture fluid was macroscopically citrine yellow in 96.0% of cases. The tuberculin skin test (TST) was performed in 4 patients (16.0%) with a positivity rate of 100.0% against 63.7% for GenXpert and ADAs (adenosine deaminase) performed in 7 patients (28.0%). Tuberculosis pleural was found in 17 patients (68,0%), peritoneal forms in 10 patients (40,0%) and pericardial forms in 4 patients (16.0%). We had 18 cases of multifocal tuberculosis (72,0%). The RHZE protocol was used in all patients for 2 months then the RH protocol for 4 months. Three patients received corticosteroid therapy (12,0%). One case of death has been recorded (4.0%).
