Résumé

Introduction : Les pathologies bucco-dentaires sont fréquentes chez les enfants et elles peuvent être révélatrices de plusieurs pathologies génétiques, environnementales et systémiques. Des relations étroites entre les maladies rénales et particulièrement les maladies génétiques et/ou congénitales altérant la fonction du rein et la formation de l’émail sont décrites dans la littérature depuis les années 70. L’objectif de notre étude était d’évaluer l’état bucco-dentaires des enfants.
Matériels et méthodes : Nous avons mené une étude transversale descriptive de juin 2020 à mai 2021 au sein du service de pédiatrique de l’HALD. Nous avons colligé 154 enfants atteints de maladie rénale.
Résultats : Les tranches d’âge entre 6 et 10 ans et entre 11 et 15 ans étaient les plus représentées et presque la moitié des patients ont été diagnostiqués de la maladie rénale entre l’âge de 6 et 10 ans ; pour la plupart (94,42%) la maladie rénale avait une origine acquise et un peu plus de la moitié (52,6%) avaient déjà un suivi de plus d’une année. Le CAO moyen et le cao moyen était respectivement de 2,59 et 3,52, montrant qu’il existe une faible activité carieuse chez les patients de notre échantillon tandis que plus de 2/3 de notre population présentaient des anomalies dentaires. Les anomalies d’éruption, de position et de structure concernaient chacune environ un tiers des patients.
Conclusion : La prise en charge des pathologies bucco-dentaires chez des enfants atteints de pathologies rénales nécessite une motivation et l’enseignement de l’hygiène bucco-dentaire, une bonne observation des lésions des muqueuses et de la structure des dents, la détection des anomalies orthodontiques et la mise en place de traitement d’interception

Abstract
Oral health status of children with renal disease in the paediatric unit of Aristide Le Dantec Hospital in Dakar

Introduction: Oral pathologies are frequent in children, and they can be indicative of several genetic, environmental and systemic pathologies. Close relationships between kidney diseases and particularly genetic and/or congenital diseases altering kidney function and enamel formation have been described in the literature since the 1970s. The objective of our study was to assess the oral health of children.
Materials and methods: We conducted a descriptive cross-sectional study from June 2020 to May 2021 in the paediatric department of the HALD. We collected 154 children with renal disease.
Results: The age groups between 6 and 10 years old and between 11 and 15 years old were the most represented and almost half of the patients were diagnosed with kidney disease between the ages of 6 and 10 years old; for the most part (94.42%) the kidney disease had an acquired origin and a little more than half (52.6%) already had a follow-up of more than one year. The average CAO and the average cao were respectively 2.59 and 3.52, showing that there is a low carious activity in the patients of our sample while more than 2/3 of our population presented dental anomalies. Abnormalities of eruption, position and structure each concerned approximately one third of the patients.
Conclusion: The management of oral pathologies in children with kidney disease requires motivation and the teaching of oral hygiene, good observation of lesions of the mucous membranes and the structure of the teeth, the detection of orthodontic anomalies and the implementation of interceptive treatment.
